
One such client of our home care program for 19 years, a 92 year old Spanish speaking, blind man with several other health problems and no family in the United States, has received intensive case management care from a team of BJCC staff members as previous efforts to address issues through normal channels and multiple agencies have not been successful.
Staff enrolled in weekly food deliveries and arranged for a NYLAG attorney to assist via the telephone to update a Health Care Proxy and develop a Living Will. Our housing advocate is has escalated efforts to get the landlord to make the necessary repairs needed to make this client’s apartment safe and rodent free. Another member of the team is arranging for a Pre-Paid Burial Plan so that his every wish can be fulfilled. Proper home care continues to be provided, He has had the same BJCC home care worker for over 13 years

J is a 73 year old male son of Holocaust survivors. He has lived in the Bronx his entire life and is known to BJCC for over 20 years. During these years, BJCC has helped him with his annual benefit and entitlement recertifications, financial assistance and supportive counseling. J has a long history of mental health issues and substance abuse and is in treatment for both.
On March 27, 2020 a fire destroyed his apartment making it uninhabitable. He was placed in a motel and then moved to an SRO while his apartment underwent repairs. During this time, BJCC social workers have continued to provide weekly supportive counseling, as well as advocating with his other providers in order to make sure our client continues to receive the services he requires.
J has been told that his apartment is almost ready for his return. The process of moving back home and replacing his belongings will be very difficult but he knows BJCC staff will be there to help support him every step of the way.
